What should the list include?
No login requiredTo make the list useful for you — and easy for healthcare professionals to understand when needed — it should include:
- Name and date of birth (optionally national ID number if you choose)
- Name of the medication and preferably the active ingredient
- Dosage form (tablet, capsule, liquid, patch, inhaler, etc.)
- Strength (for example mg, micrograms, IU)
- Dosage and schedule (how and when you take the medication)
- Reason for use (indication)
- Other relevant products (vitamins, supplements, herbal products)

Practical tip: Add a “Note” for each medication (e.g. “take with food” / “as needed”).
